2009 Sports Physicals
Kalkaska Memorial Hospital, in cooperation with the Teen Health Center & the Rural Health Clinic will be offering sports physicals this year on an individual basis.
To encourage athletes to obtain their physical in a timely manner
throughout the summer Kalkaska Memorial Hospital will offer them
during June and July for $5.00. In
August the charge will be $20. (Free physicals are not
available this year).
If athletes are coming with a parent, physical cards will be
available at the hospital and parental consent will be obtained at
that time. Athletes going without their parent need to bring
the physical card completed and signed by their
parent.
Kalkaska Memorial Hospital will be billing athlete’s insurance, if
applicable. If athletes bring their $5 with them, KMH will not hold
athletes responsible for a co-pay or deductible.
Athletes need to call 258-7791 to schedule an
appointment.
These physicals are for students who will be in grades
6th – 12th for the 2009-10 school year.
Physical forms may be picked up at the
Cherry Street, Middle School, Rapid City or High School office.
