Student Meals

Household Applications for Free and Reduced Priced School Meals and Household Information Surveys are available at all school offices or can be found on our district website. Applications and Surveys may be filled out any time during the school year. We are asking that you please fill out and sign the Household Information Survey and get it back to one of the school offices. The survey is for administrative purposes only and is not to determine eligibility. This survey allows our district to benefit from various State and Federal supplemental programs like Title I A, At Risk (31a), Title II A, E-Rate, etc. This survey is critical in determining the amount of money the district receives from a variety of supplemental programs. We are asking that you please complete and submit it as soon as possible. Also, please complete your household application as soon as possible. Previous year benefits will only be in effect for 30 school days after the first day of school. If an application is not received by the Food Service Office before October 11, 2016 your child’s meal status will change to full pay. If a student selects milk ONLY, there is a $0.50 charge. If a student selects a second entrée, there is a $1.75 charge. As a reminder, students are required to select a fruit and/or vegetable with lunch for the meal to be considered a full meal. Anything less than a full meal is charged at a la carte prices.

Allergies

If a student has FOOD ALLERGIES, a Medical Statement Form must be completed by a physician to accommodate the child’s needs before any substitutions can be made. Forms are available at any school office or on our website at kpschools.com.
